• 1. Confirmation of a child care space will be made by the child care administration.
• 2. Upon your acceptance a financial commitment of $150 will be required (applied to your last month’s fees upon submission of 30 days’ notice).

I have read and understood the above agreement

I authorize the child care provider/staff to obtain the following services for this child if necessary: Public Health Nurse, Physician and/or Ambulance in the event of an emergency. (Ambulance fees and/or health care costs are the responsibility of the parent/guardian).

I give permission for my child to be photographed or videotaped during a Blind Bay Munchkins Event. I understand that these pictures may be used in displays, websites, and/or published.

I give permission for my child to participate in spontaneous walks to local parks, schools and daily outside play within the Sorrento area with Blind Bay Munchkins Staff.

I give permission for my child to be photographed or videotaped during a Shuswap Explorers Event. I understand that these pictures may be used in displays, websites, and/or published.

I give permission for my child to be photographed or videotaped during a The Outsiders Event. I understand that these pictures may be used in displays, websites, and/or published.

I give permission for my child to participate in spontaneous walks to local parks, schools and daily outside play within the Sorrento area with Shuswap Explorers Staff.

I give permission for my child to participate in spontaneous walks to local parks and schools and daily outside play within the Shuswap with The Outsiders Staff.

I hereby give my consent for Shuswap Explorers to transport my child to and from pro-d day camps (Salmon Arm and area) and give my consent for transportation to field trips that occur during the time my child is in care at Shuswap Explorers. I understand that my child will be traveling by bus or vehicle.

I hereby give my consent for The Outsiders to transport my child to and from school and give my consent for transportation to field trips that occur during the time my child is in care at The Outsiders. I understand that my child will be traveling by bus.
